fix(channels): handle io.ReadAll error in Discord adapter

DiscordAdapter.SendMessage ignored the error from io.ReadAll on the response body. When the read failed on an error status, the empty body meant the webhook error response lost its diagnostic detail (Discord error messages are in the body). Surface the read error explicitly.
